South Dakota Medicaid is seeking a highly motivated individual to serve as a leader in our Care Management Program as DSS strives to add value to SD Medicaid and evaluates enhancements need to our current PCP and Health Home programs.
  • Execute routine scheduled processes required of the Care Management programs (PCP, Health Home, & Baby Ready) as needed.
  • Assist with initiatives related to existing Health Home Program including but not limited to satisfying program reporting requirements; assisting in developing and conducting training opportunities for Health Home Care Coordinators; creating and implementing quality improvement projects.
  • Regularly communicate with vendors, stakeholders, and interested parties to ensure quality assurance, data metrics and reporting, and various external obligations are satisfied as required.
  • Assist in creating and/or editing contracts with multiple vendors regarding various aspects of the Care Management programs, including, but not limited to Dual Special Needs Plans; quality review audits; performance outcome measure metrics; and data processing.
  • Research and design enhancements to the state's PCCM program to provide effective care management, including evaluating and recommending changes to the State plan as it relates to enhancing our current PCP program; data and population health analysis of current Medicaid recipients and Expansion recipients; conduct a cost benefit analysis; create and project the impact to the South Dakota Medicaid budget; researching best practices in care management and program design and evaluating application to South Dakota; making recommendations to DSS leadership regarding program design and enhancements for the PCCM program; designing a quality program for PCCM program, evaluate and recommend IT and vendor procurement.
  • Lead the process to implement enhancements to the PCCM program including but not limited to forming and leading a stakeholder group; creating an application process; conducting outreach regarding the program details; recruiting and educating providers and clinics; designing the attribution process for recipients; defining and leading quality initiatives; working with the IT and Data team to make any necessary enhancements; provide technical assistance and training to DSS staff.

Licenses and Certifications:

Prefer a Bachelors degree in health care administration, business, health information management, public health, or a similar degree

or

2 or more years of experience in a related field.

Knowledge, Skills, and Abilities:
  • analyze and evaluate program plans, and make recommendations for program operation improvement and cost effectiveness of the Care Management programs;
  • understanding of or ability to learn Department data and program interface applications (MMIS, Portal applications, BEES, etc...);
  • provide leadership, guidance, and direction and establish and maintain effective working relationships with federal, state, local, and other departmental officials, and public and private agencies;
  • plan, direct, and develop initiatives and recommendations for program and budgetary modifications for new state initiatives;
  • communicate information clearly and concisely;
  • experience with various project management;
  • ability to work with complex systems, environments, and understand business needs;
  • strong problem solving skills and ability to move in fast paced environment;
  • skilled user of Microsoft Office suite;
  • demonstrated success exercising sound judgment;
  • experience in successfully leading large and smaller teams with a variety of knowledge, skills, and abilities.
